摘要:
目的探讨血清黏蛋白1(MUC1)和中性粒细胞胞外诱捕网(NET)在急性肺栓塞(APE)危险分层中的表达以及对预后的评估价值。方法选择2022年11月至2024年1月在邢台市中心医院就诊的298例APE患者,根据危险分层分为低危、中危和高危三个亚组,并根据入院后28 d的预后情况分为存活组和死亡组。收集所有患者的临床资料,检测MUC1和NET并进行统计学分析。结果死亡组和存活组血清MUC1分别为(8.44±1.26)U/mL、(5.67±1.05)U/mL,血清NET分别为(40.45±6.21)U/mL、(2.38±5.44)U/mL,死亡组均显著高于存活组(P<0.01);低危、中危和高危患者的血清MUC1分别为(4.92±0.88)U/mL、(6.24±1.38)U/mL、(8.56±1.22)U/mL,血清NET分别为(27.28±3.66)%、(35.97±4.83)%、(39.68±3.27)%,差异均具有统计学意义(P均<0.01),随着危险分层升高,血清MUC1和NET显著升高(P<0.01);血清MUC1和NET与肺栓塞危险分层之间存在显著正相关(r=0.565,P<0.001;r=0.630,P<0.002)。经多因素Logistic回归分析,D二聚体、氨基末端脑利钠肽前体(NTproBNP)、纤维蛋白原、MUC1、NET为肺栓塞患者死亡的独立危险因素(P<0.05);MUC1、NET对急性肺栓塞预后评估的ROC曲线下面积(AUC)分别为0.812(95%CI:0.751~0.874)、0.864(95%CI:0.809~0.919),MUC1和NET联合诊断的AUC为0.877(95%CI:0.820~0.934)。结论血清MUC1和NET与APE危险分层之间存在显著相关性,对APE患者的死亡评估具有较高的评估价值。
Abstract:
ObjectiveTo explore the expression and the prognostic value of serum mucin1 (MUC1) and neutrophil extracellular traps (NET) in risk stratification of acute pulmonary embolism (APE).Methods298 APE patients who visited at Xingtai Central Hospital from November 2022 to January 2024 were divided into three subgroups based on risk stratification:lowrisk, moderate risk, and highrisk. They were also divided into a survival group and a death group based on their prognosis 28 days after admission. Clinical data from all patients were collected, and MUC1 and NET were detected and subjected to statistical analysis.ResultsThe serum levels of MUC1 of death group and survival groups were (8.44±1.26)U/mL, (5.67±1.05)U/mL, and the NET of death group and survival groups were (40.45±6.21)U/mL, (32.38±5.44)U/mL, The levels in the death group were significantly higher than those in the survival group (P<0.01);The serum MUC1 levels of lowrisk, moderate risk, and highrisk group were (4.92±0.88)U/mL, (6.24±1.38)U/mL, and (8.56±1.22)U/mL, respectively. The serum NET levels of lowrisk, moderate risk, and highrisk group were (27.28±3.66)%, (35.97±4.83)%, and (39.68±3.27)%, respectively. All difference showed statistically significant (P<0.01). As the risk stratification increased, serum MUC1 and NET levels significantly increased (P<0.01);There was a significant positive correlation between serum MUC1, NET and pulmonary embolism risk stratification (r=0.565, P<0.001;r=0.630, P<0.002). Ddimer, Namion terminal brain natriuretic peptide precursor (NTproBNP), fibrinogen, MUC1, and NET were identified as independent risk factors for mortality in patients with pulmonary embolism by multiple Logistic regression analysis (P<0.05); The area under the ROC curve of MUC1, and NET for prognostic evaluation of acute pulmonary embolism were 0.812 (95%CI: 0.751-0.874) and 0.864 (95%CI: 0.809-0.919), respectively, the area under the ROC curve for the joint diagnosis of MUC1 and NET was 0.877 (95%CI: 0.820-0.934).ConclusionThere is a significant correlation between serum MUC1 and NET and APE risk stratification, which has high evaluation value for the mortality assessment of APE patients.
