目的 探讨脑利钠肽(BNP)对老年心力衰竭患者的预后价值。方法 分别测定62例老年心衰患者入院及出院前的血浆BNP及心脏彩色多普勒参数，随访出院后患者主要心血管事件的发生情况。结果 在未发生心血管事件组，入院出院BNP水平分别为(1 018.77±1 187.767)pg/ml、(184.46±195.867)pg/ml,P＝0.000；在发生心血管事件组，入院出院BNP水平分别为(1 117.45±942.554)pg/ml、(1 132.91±1 440.31)pg/ml,P＝0.965，其中7例患者BNP水平较入院时升高，余10例患者BNP水平较入院时明显降低(P=0.03)，但均未降至正常。结论 动态观察BNP有利于判断心衰患者预后，BNP水平越高预后越差，治疗后BNP反而升高者预后极差。
Abstract:
Objective To explore the value of brain natriuretic peptide (BNP) in the prognosis of heart failure in elderly patients. Methods The plasmas levels of BNP and echocardiography parameters were measured in 62 elderly patients with heart failure on admission and discharge. All patients were followed up to investigate the clinical main adverse cardiac events (MACE). Results In the Non-MACE group, the plasma levels of BNP were (1 018.77±1 187.767) pg/ml and (184.46±195.867) pg/ml on admission and discharge respectively, P=0.000. In the MACE group, the levels of BNP were (1 117.45±942.554) pg/ml and (1 132.91±1 440.31) pg/ml on admission and discharge respectively,P＝0.965. In the second group, the levels of BNP were elevated in 7 patients, and significantly degraded in other 10 patients (P=0.03), but did not reduce to normal.Conclusion To observe the changes of the plasma BNP is beneficial to the prognosis of heart failure: the higher the level of BNP, the worse of prognosis. The prognosis is worst when the plasma of BNP elevated after therapy.
