目的 探讨阻塞性睡眠呼吸暂停低通气综合征(obstructive sleep appea-hypopnea syndrome, OSAHS)患者血浆食欲素A(Orexin-A)变化，以及经鼻持续气道正压通气(nasal continuous positive airway pressure,nCPAP)治疗对其影响。方法 选取中度、重度OSAHS患者33例，另选取单纯肥胖者28例作为对照组，采用多道睡眠图仪对所有对象进行夜间睡眠监测，记录暂停低通气指数(AHI)、微觉醒指数(MAI)、最低血氧饱和度(LSaO2)和平均血氧饱和度(MSaO2)，并用放射免疫法测定血浆食欲素A水平。结果 OSAHS患者血浆食欲素A水平与AHI、MAI呈正相关，与夜间LSaO2、MSaO2呈负相关。治疗前OSAHS患者血浆食欲素A水平明显高于对照组，nCPAP治疗后AHI、MAI及LSaO2、MSaO2明显改善，且食欲素A较治疗前明显降低。结论 血浆食欲素A可作为评估病情、判断疗效的指标。
Abstract:
Objective To explore the changes in plasma orexin A levels in patients with obstructive sleep apnea hypopnea syndrome (OSAHS ) and the effect of nasal continuous positive airway pressure (nCPAP) treatment. Methods 33 cases of OSAHS and 28 cases of control were examined by polysommography (PSG) to monitor the night sleep. AHI，MAI，LSaO2 were recorded and the levels of orexin A were determined by radioimmunoassay. Results Plasma orexin A levels in patients with OSAHS positively correlated with AHI and MAI (r=0.495, 0.571), but negatively correlated with LSaO2 and MSaO2 . After CPAP treatment, AHI, MAI, LSaO2, and MSaO2 were much better than before, and the levels of orexin A significantly decreased. Conclusion Orexin A can be an index to assess the disease and therapy effect.
