摘要:
目的探讨重度烧伤复合肺爆震伤的临床特点及总结诊疗中的经验教训。方法对31例重度烧伤复合肺爆震伤患者的临床表现、治疗措施及疗效进行归纳分析。结果31例患者治 愈。入院时患者心肺功能障碍，尤以肺脏损伤为重，肝肾功能正常，血浆D-二聚体显著异常，血小板计数正常。19例呼吸功能不全、低氧血症的患者，给予气管切开、保护性机械 通气，尽快纠正低氧血症。并发肺部感染16例, 脓毒症5例，急性呼吸窘迫综合征4例，心功能衰竭4例。结论烧伤复合肺爆震伤伤情重，多器官功能受损，临床过程复杂，要早期诊 断和救治。早期气管切开、机械通气及脏器功能保护是影响预后的重要措施，综合治疗措施是成功救治的关键。
Abstract:
ObjectiveTo explore clinical characteristics of severe burns combined with blast injury of the lungs to sum up the experience and lessons of management and diagnosis. MethodsThe clinical manifestations, management methods and effectiveness of 31 patients with severe burs combined blast injury of the lungs were summarized and analyzed. ResultsAll 31 patients were cured. The laboratory results of these patients on admissin showed that the heart and pulmonary function were abnormal, especially pulmonary dysfunction. The functions of liver and kidney were normal. D-dimer level of blood plasma was strikingly abnormal, while platelet count were within normal limits. 19 cases with acute lung injury and hypoxemia were received immediate tracheotomy and protective ventilation to correct hypoxemia. There were 16 cases complicated with pulmonary infection, 5 cases with sepsis, 4 cases with acute respiratory distress syndrome, and 4 cases with heart failure. ConclusionSevere burns combined with blast injury of the lungs are often accompanied with multiple organ damage. The clinical process is so complicated that it is necessary to achieve early diagnosis and early treatment. Early broncheotomy, mechanical ventilation and the protection of organ function is an important prognostic measures, and combined therapy is the key to successful treatment.
