摘要:
目的探讨非胸腔镜辅助下改良的Nuss手术矫治漏斗胸的初步经验。方法2009年2月至2010年3月，采用改良的Nuss手术矫治漏斗胸23例，年龄2.5~15岁，平均6.5岁，所有病例均在非胸腔镜辅助下完成。21例为对称型，2例为非对称型。结果23例顺利完成手术，术中平均出血<20 ml，5例出现少量气胸及皮下气肿，保守治疗后消失。术后平均住院6.2 d，矫形效果21例为优，2例良。所有患者术后均获随访，随访1~14个月，均无明显不适，无钢板移位。结论非胸腔镜辅助下改良的Nuss手术安全易行，创伤小，手术时间短，术后恢复快，近期矫形效果满意，远期效果待进一步观察。
Abstract:
ObjectiveTo explore the early experience of modified Nuss procedure for pectus excavatum without the assistance of thoracoscopic surgery. MethodsTwenty-five pectus excavatum patients were treated by modified Nuss procedure without thoracoscope from Februrary 2009 to March 2010. The age of patients ranged from 2.5 to 15 years and the average age was 6.5 years. All patients were performed without the assistance of thoracoscope. Twenty-one patients were symmetic pectus excavatum and 2 patients were asymmetric pectus excavatum. ResultsThe operation in all patients were successfully accomplished with the operative time ranging from 35 to 62 minutes, and the average blood loss was less than 20 ml. Pneumothorax and pneumoderma occurred in 5 patients, and all resolved by expectant treatment. The average hospital length of stay was 6.2 days. Orthopaedic results evaluation showed that excellent in 21 patients and good in 2 patients. All patients were followed up in 1-14 months without any complaints, and there were no bar displacement. ConclusionModified Nuss procedure without thoracoscope is easy to perform. It is safe and minimally invasive, and characterized by short operative time and fast recovery. It has a satisfactory short-term result. However, the long-term results require to be observed further.
