摘要:
目的 探讨Schatzker Ⅵ型胫骨平台骨折患者长途转送途中骨筋膜室综合征的预防。方法 回顾性分析2003年6月至2010年6月采用外固定支架、石膏和真空夹板固定并进行长途转送的SchatzkerⅥ型胫骨平台骨折63例的病例资料，其中外固定支架组24例、石膏组19例、真空夹板组20例。对比三组患者长途转运前后的X线片情况及骨筋膜室综合征发生情况。结果 外固定支架组的骨筋膜室综合征发生率明显低于石膏组和真空夹板组(P<0.05)，石膏组和真空夹板组之间无统计学差异(P>0.05)；外固定支架组骨折移位发生率明显低于石膏组和真空夹板组(P<0.05)，石膏组和真空夹板组之间无统计学差异(P>0.05)。结论 外固定支架固定SchatzkerⅥ型胫骨平台骨折，能有效控制损伤，提高患者长途转运的安全性，防止骨筋膜室综合征发生。
Abstract:
Objective To investigate the methods of preventing compartment syndrome of Schatzker Ⅵ tibial plateau fractures in patients with long-distance transport.Methods A retrospective analysis of clinical data of 63 patients with Schatzker Ⅵ tibial plateau fractures from June 2003 to June 2010 using the external fixator,plaster and vacuum splint who make long-distance transfer,including 24 cases with external fixation,19 cases with gypsum,20 cases with vacuum splint.And to compare the difference of the three methods of immobilization in the changes of radiography and the incidence of compartment syndrome anterior and posterior long-distance transfer.Results The incidence of compartment syndrome in external fixation group was significantly lower than that in plaster splint group and vacuum group (P<0.05),witch was not significant different between vacuum and plaster splint groups (P>0.05); The incidence of fracture displacement in external fixation group was significantly lower than that in plaster splint group and vacuum group (P<0.05),and while there was no significant difference between vacuum and plaster splint groups (P>0.05).Conclusion The external fixation,which is used to stabilize Schatzker Ⅵ tibial plateau fractures,can be effective in damage control,preventing compartment syndrome,and also be able to improve patients' safety in long-distance transport.
