目的 通过对OSSTEM短种植体在磨牙区临床应用3年的临床观察,评估其临床应用价值。方法 选取2010年6月-2011年6月行种植修复的76例患者,共植入113颗OSSTEM TS Ⅲ型(韩国奥齿泰TS Ⅲ型种植系统)短种植体,全部种植位点位于下颌磨牙区。测量评估C/I比值(冠与种植体长度比值)、种植体颈部近远中牙槽骨高度及临床指征,采用Zarb种植体成功标准和Wheeler种植体存留率标准评价OSSTEM TS Ⅲ短种植体3年成功率和存留率。结果 75个种植位点采用单冠修复,38个位点采用联冠修复。113颗OSSTEM TS Ⅲ短种植体负重后经36个月的临床观察,C/I比值平均值为1.25,种植术后即刻、1年、3年种植体近中边缘骨高度分别为(0.55±0.08)mm、(-0.07±1.09)mm、(-0.28±1.18)mm,远中边缘骨高分别为(0.59±1.19)mm、(-0.21±1.13)mm、(-0.28±1.36)mm,OSSTEM TS Ⅲ短种植体3年存留率100.0%,成功率93.8%。结论 在下颌磨牙区植入短种植体进行种植修复可以取得令人满意的临床效果,与常规种植体并无差异。长期效果有待进一步研究。
Abstract:
Objective To evaluate the clinic importance of OSSTEM short implants placed in mandibular molar areas by observing its clinic curative effect in three years. Methods From June 2010 to June 2011, 113 OSSTEM short implants in 76 patients were consecutively placed in mandibular molar areas. The mesial and distal marginal bone height around implants and the crown-implant ratio (C/I) of all sites were measured and the data were collected. The success ratio and survival ratio were accessed with Zarb's successful criteria and Wheeler's survival criteria. Results 75 implant sites were rehabilitated with single crown and 38 sites with united crown and all loaded for 36 months, which mean C/I was 1.25. The mesial bone height around the implants at the times immediately after placing implant, after loading for 12 months and after loading for 36 months was (0.55±0.08) mm, (-0.07±1.09) mm, (-0.28±1.18) mm respectively; and the distal bone height was (0.59±1.19) mm, (-0.21±1.13) mm, (-0.28±1.36) mm respectively. The 3 year's success rate and survival rate were 93.8% and 100.0%. Conclusion The results of this 3-year retrospective study show that the short-term clinic curative effect of short dental implant placed in mandibular molar areas was satisfactory and has no significant difference with the reported normal implant. However, the long-term results are warranted in the future.
