摘要:
胃癌在全世界范围的发病率居男性第四位和女性第五位,且大部分新病例发生在东南亚国家,为重要公共卫生问题。尽管外科手术为主的综合治疗是目前胃癌治疗有效手段,但不同国家术后5年生存率差异性很大。胃癌的发生、发展及转移是一个复杂的生物学过程,受到遗传、环境、饮食等多种因素的影响,体现了“变异-选择-适应”的进化过程。胃癌术后复发和转移是患者死亡的主要原因,其分子机制尚不明确,但这一过程可能与胃癌发生的机制相似,通常由宿主免疫系统失衡,炎症相关信号网络在癌旁组织中激活引起。胃癌的炎症相关恶性转化大多是由病原体感染驱动的,是多种细胞、基因、非编码RNA等共同参与的生物学过程。以免疫调控的炎症网络为切入点,运用系统生物学方法,结合流行病学队列研究,从胃癌预后相关高通量基因标志谱中挖掘、整合和评价关键分子事件,开发有效性更高的预测预后标志群,同时结合临床及病理学信息进行预测,对于加快胃癌的个体化治疗、改善病人预后具有重要意义。
