摘要:
目的 通过观察相关炎症因子在不同程度炎症声带黏膜表达的差异,探讨其对声带黏膜屏障损伤的作用。方法 将26例确诊伴有咽喉反流(LPR)的声带白斑且拟行手术治疗的患者随机分为质子泵抑制剂(PPI)组(口服艾司奥美拉唑镁肠溶片)和对照组(口服胃黏膜保护剂),每组13例,分别采集手术中及术后8周的声带黏膜标本。用HE染色对声带炎症的病理改变进行评分比较;ELISA法测定炎症因子TNF-α、IL-1β、IL-6在不同程度炎症黏膜的表达水平。所有患者每月定期复查,随访时间至少为12个月。结果 术后12个月随访,对照组和PPI组累计复发率分别为84.6%和38.5%,累计复发率差异有统计学意义(P<0.05)。PPI组术后8周声带黏膜炎症病理评分均值较术中黏膜评分明显降低(P<0.01);对照组术中声带黏膜炎症病理评分均值与术后8周评分均值相比,差异无统计学意义(P>0.05)。ELISA检测显示,PPI组术后8周声带黏膜IL-1β、TNF-α、IL-6表达与术中比较显著下调(P<0.01);而对照组术中及术后8周声带黏膜IL-1β、TNF-α、IL-6表达差异无统计学意义(P>0.05)。结论 咽喉反流物刺激声带黏膜的炎症反应可能是声带黏膜损伤的重要机制。
Abstract:
Objective To study the roles of related inflammatory factors in vocal cord mucosal barrier damage via probing the differential expression of these factors in vocal cord mucosa with inflammation of different degrees.Methods Twenty-six patients suffering from vocal leukoplakia accompanied with laryngopharyngeal reflux (LPR) were randomly divided into groups with or without oral administration of proton pump inhibitor (PPI), 13 cases in each group. Mucosal specimens of vocal cord were collected from all the patients during operation and at 8 weeks after operation. HE staining was used to score the pathological changes of the mucosa. ELISA was utilized to detect the levels of inflammatory factors including TNF-α, IL-1 and IL-6.Results The accumulated recurrence rates at 12 months after operation of PPI-treated group and the control group were 38.5% and 84.6% respectively, and the difference was statistically significant (P<0.05). In the the control group, the average inflammation score evaluated by HE staining in specimens obtained 8 weeks after operation was significantly lower than those obtained during operation, and the difference was statistically significant (P<0.01). In the control group, the difference was statistically insignificant (P>0.05). As for IL-1β, TNF-α and IL-6, their expression in the specimens of 8 weeks after operation in the PPI-treated group were down-regulated when com-pared with those in the specimens obtained during operation (all P<0.05), while their differences in the control group were statistically insignificant (all P>0.05).Conclusion The possible mechanism of vocal mucosal barrier damage in vocal leukoplakia accompanied with LPR maybe the vocal mucosal inflammation induced by refluxed materials.
