摘要:
目的 探究富氢液对于肝移植手术患者腔静脉阻断期肾功能的保护作用。方法 选取60例肝移植术患者,随机将其分为对照组(n=30)和富氢液组(n=30),麻醉诱导后分别经胃管注射250mL的等渗盐水和富氢液。于麻醉诱导(T0)、新肝期(T3)以及术毕(T5)时分别采集2组患者的静脉血和尿液标本,并检测血清尿素氮(BUN)、血清肌酐(Cr)、肌酐清除率、白细胞介素-18(IL-18)、半胱氨酸蛋白酶抑制剂(Cystain C)、肾损伤分子-1(KIM-1)以及中性粒细胞明胶酶相关载脂蛋白(NGAL)等指标。结果 2组患者在T3和T5期血清Cr和BUN浓度较T0期升高,肌酐清除率下降,差异有统计学意义(P<0.05);与对照组比较,富氢液组患者的血清Cr[(164.8±18.4)μmol/L vs (116.7±14.8)μmol/L]、BUN浓度[(8.0±2.7)mmol/L vs (5.8±1.3)mmol/L]明显降低(P<0.05),肌酐清除率[(73.2±20.1)mL/min vs (86.3±18.4)mL/min]明显升高(P<0.05)。与对照组相比,富氢液组IL-18、Cystain C、KIM-1以及NGAL含量在T0期差异无统计学意义(P>0.05),但在T3和T5期表达量均显著下降(P<0.05)。结论 富氢液对于肝移植手术患者腔静脉阻断期肾功能有一定的保护作用。
Abstract:
Objective To investigate the protective effect of hydrogen-rich liquid on renal function in liver transplantation with the blocking period of vena cava.Methods 60 patients with liver transplantation were randomly selected and divided into control group (group C, n=30) and hydrogen-rich group (group H, n=30). After induction of anesthesia, 250 mL of isotonic saline and hydrogen-rich liquid were injected via gastric tube respectively. During anesthesia induction (T0), new liver stage (T3) and post-operation (T5), venous blood and urine samples of 2 groups were collected. urea nitrogen (BUN), serum creatinine (Cr), creatinine clearance rate, interleukin-18 (IL-18), cysteine protease inhibitors (Cystain C), renal injury molecule-1 (KIM-1) and neutrophil gelatinase-related apolipoprotein (NGAL) were measured.Results Compared with the T0 phase, the levels of Cr and BUN increased and the creatinine clearance rate decreased in T3 and T5 phases in the two groups. At the same time, the serum Cr [ (116.7±14.8) μmol/L vs (164.8±18.4) μmol/L\] and BUN concentrations [(5.8±1.3) mmol/L vs (8.0±2.7) mmol/L\] and creatinine clearance rate [(86.3±18.4)mL/min vs (73.2±20.1) mL/min\] in the H group were better than those in the C group, and the difference was statistically significant (P<0.05). Compared with group C, the levels of IL-18, Cystain C, KIM-1 and NGAL in group H were not significantly different (P>0.05) in T0, but the expression levels of IL-18, Cystain C, KIM-1 and NGAL were significantly decreased in T3 and T5 (P<0.05).Conclusion Hydrogen-rich fluid has certain protective effect on renal function in liver transplantation with the blocking period of vena cava.
