摘要:
目的 基于美国放射学会乳腺超声报告与数据系统(BI-RADS-US)评分标准,采用联合超声造影对乳腺肿块进行分类,比较其与单纯常规超声分类的差异;并评价超声造影对乳腺良恶性肿块的鉴别诊断能力。方法 收集247例经病理证实的乳腺肿块,分别采用常规超声和联合超声造影对肿块进行BI-RADS-US分类,采用χ2检验评价两者间差异。参照病理结果 ,采用受试者工作特征曲线,比较两种方法 对乳腺良恶性肿块的鉴别诊断能力。结果 在常规超声基础上增加超声造影检查,对乳腺肿块的分类发生变化,表现为3类和5类病例数增多(3类增加40例、5类增加49例)、4类病例数减少(4a减少76例、4b减少7例、4c减少4例);常规超声和联合超声造影对乳腺肿块的BI-RADS-US分类差异有统计学意义(P<0.01);联合超声造影对良恶性肿块的鉴别诊断能力优于常规超声(曲线下面积分别为0.908 vs 0.722, P<0.01)。结论 在常规超声对乳腺肿块分类的基础上,联合超声造影技术,能提高对乳腺肿块分类的准确性,提高对良恶性肿块的鉴别诊断能力。
Abstract:
Objective Based on breast imaging reporting and data system for ultrasonography (BI-RADS-US), this study aims to compare the difference of classification for breast masses between ultrasound (US) and combined contrast-enhanced ultrasound (CEUS). To evaluate the efficiency of combined CEUS in differential diagnosis of benign and malignant masses.Methods Total 247 cases of breast masses were enrolled in this approved retrospective study. All masses were classified according to US and combined CEUS respectively, based on BI-RADS-US. Chi square test was used to analyze the difference of classification between US and combined CEUS. The diagnostic efficacy of combined CEUS in terms of sensitivity and specificity were analyzed by receiver operating characteristic (ROC) curve analyses.Results After combined with CEUS characteristics, the changes of BI-RADS-US classification were mainly manifested by an increase in class 3 and class 5 (40 cases in class 3, 49 cases in class 5) and a decrease in class 4 (76 cases in class 4a, 7 cases in class 4b, and 4 cases in class 4c).There was statistical difference in classification for breast masses between US and combined CEUS(P<0.01). The area under the curve (AUC) was 0.908 for combined CEUS and 0.722 for US(P<0.01).Conclusion On the basis of conventional US BI-RADS-US classification, the combination of CEUS is helpful to improve the evaluation ability of BI-RADS-US classification for breast masses.
