摘要:
目的 探讨超声检测妊娠期高血压(HDCP)患者胎儿心室收缩功能对评估胎盘缺氧程度的作用。方法 选取解放军联勤保障部队第九一○医院2014年1月至2018年1月收治的476例HDCP患者为研究对象,按美国妇产科医师协会(ACOG)的HDCP分型标准分为3个亚组:妊娠期高血压组(255例)、子痫前期轻度组(149例)和子痫前期重度组(72例)。另选择医院同期血压正常的孕产妇173例为对照组。彩色超声诊断仪检查并记录胎儿左、右心收缩功能指标:左心室心肌收缩功能(LVSF1)、右心室心肌收缩功能(LVSF2)、左室收缩功能(RVSF1)、右室收缩功能(RVSF2);检测脐带静脉血缺氧相关指标水平:缺氧诱导因子-1α(HIF-1α)、一氧化氮(NO)、热休克蛋白70(HSP70);检测胎盘活性氧(ROS)含量和抗氧化物质超氧化物岐化酶(SOD)、谷胱甘肽过氧化物酶(GSH-PX)和过氧化氢酶(CAT)含量。结果 HDCP3个亚组胎儿左、右心室收缩分数LVSF1、LVSF2、RVSF1、RVSF2均显著高于对照组(P<0.05);HDCP3个亚组胎儿脐静脉血HIF-1α、HSP70水平显著高于对照组(P<0.05),胎盘ROS水平显著高于对照组(P<0.05),胎盘SOD、GSH-Px、CAT水平显著低于对照组(P<0.05)。胎儿心室收缩功能参数LVSF1、LVSF2、RVSF1、RVSF2与脐静脉血HIF-1α水平均存在正相关(r=0.574,0.568,0.533,0.628,P<0.05),与胎盘ROS水平呈正相关(r=0.646,0.463,0.478,0.532,P<0.05),与胎盘SOD水平呈负相关(r=-0.572,-0.487,-0.493,-0.621,P<0.05)。结论 对HDCP患者进行超声胎儿心室收缩功能检测有利于早期监测胎盘缺氧程度。
Abstract:
Objective To investigate the role of fetal ventricular systolic function in the assessment of placental hypoxia by ultrasonography in patients with hypertensive disorders complicating pregnancy (HDCP).Methods 476 patients with HDCP admitted to the 910th hospitals from January 2014 to January 2018 were studied, according to the American association of obstetrics and gynecology doctors (ACOG) HDCP classification standard. The patients were divided into three subgroups: gestational hypertension group (255 cases), preeclampsia mild group (149 cases) and preeclampsia severe group (72 cases). In the control group, 173 cases with normal blood pressure in our hospital were selected. Color ultrasonic diagnostic instrument check and record the fetus left and right heart systolic function index: left ventricular myocardial systolic function (LVSF1), right ventricular myocardial systolic function (LVSF2), left ventricular systolic function (RVSF1), right ventricular systolic function (RVSF2). The correlation indexes of umbilical cord venousblood hypoxia were detected: hypoxia inducible factor-1α(HIF-1α), NO, heat shock protein 70 (HSP70). The contents of placental reactive oxygen species (ROS) and antioxidant superoxide dismutase (SOD), glutathione peroxidase (gsh-px) and catalase (CAT) were detected.Results The left and right ventricular systolic fractions LVSF1, LVSF2, RVSF1 and RVSF2 of the three subgroups of HDCP3were significantly higher than those of the control group (P<0.05). The hif-1 q and HSP70 levels of fetal umbilical vein blood in HDCP3 subgroups were significantly higher than those in the control group (P<0.05). The level of placental ROS in HDCP group was significantly higher than that in the control group (P<0.05). The levels of placental SOD, gsh-px and CAT in HDCP group were significantly lower than those in the control group (P<0.05). Fetal ventricular systolic function parameter LVSF1, LVSF2, RVSF1, RVSF2 and umbilical venous blood HIF-1 alpha level there were positive correlation (r=0.574, 0.568, 0.574, 0.568, P<0.05), and placental ROS levels were positively correlated (r=0.646, 0.463, 0.646, 0.463, P<0.05), and negatively correlated with the placenta SOD level (r=0.572, 0.487, 0.493, 0.621, P<0.05).Conclusion Ultrasound fetal ventricular systolic function test in HDCP patients is conducive to early detection of placental hypoxia.
