摘要:
目的 探讨模拟军事训练致热损伤动物休克模型脑损伤及凝血功能异常的相关因素。 方法 根据热处理和37 ℃恒温处理方法不同,将150只健康SD大鼠按随机数字表法分为热处理30、60、90、120 min组和热处理死亡组,以及恒温(37 ℃)处理30、60、90、120 min组和恒温处理死亡组,每组15只。分析比较各组凝血功能[D-二聚体水平、凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)以及纤维蛋白原(FIB)]、血气分析指标[血液氧分压(PaO2)、二氧化碳分压(PaCO2)、pH值]、颅内压、脑灌注压,原位标记法(TUNEL)检测海马神经细胞凋亡率。 结果 热处理90、120 min组和热处理死亡组D-二聚体水平[(0.73±0.22)、(0.93±0.34)、(1.15±0.33)mg/L]均高于恒温处理同时间点组(P<0.05);热处理60、90、120 min组以及热处理死亡组PT[(16.51±1.05)、(19.79±1.97)、(27.65±2.55)、(36.28±3.88)s]、APTT[(38.87±3.30)、(49.95±7.88)、(56.47±8.62)、(63.84±9.94)s]以及TT[(22.54±1.50)、(28.74±3.93)、(36.99±4.82)、(40.18±6.26)s]均较恒温处理同时间点组明显延长(P<0.05);热处理60、90、120 min组和热处理死亡组FIB水平[(2.01±0.18)、(1.53±0.24)、(1.05±0.22)、(0.82±0.20)mg/dL]显著低于恒温处理同时间点组(P<0.05)。热处理各组大鼠PaO2、血pH随处理时间的延长呈逐渐降低趋势,PaCO2逐渐升高(P<0.05)。热处理60、90、120 min组和热处理死亡组脑含水量[(74.25±0.5)%、(80.54±0.8)%、(83.54±0.2)%、(84.41±0.3)%]、脑海马神经细胞凋亡率[(19.52±0.61)%、(20.96±0.58)%、(24.33±1.61)%、(28.20±2.74)%]明显高于恒温处理各组(P<0.05),热处理90、120 min组和热处理死亡组颅内压[(1.88±0.33)、(2.19±0.11)、(2.78±0.39)、(3.21±0.58)kPa]显著高于恒温处理同时间点组大鼠(P<0.05);热处理60、90、120 min组和自然死亡时脑灌注压[(8.25±0.71)、(6.38±0.82)、(5.59±0.69)、(4.89±0.65)kPa]均低于恒温处理同时间点组(P<0.05)。 结论 高温环境下易导致中暑,该过程影响机体凝血功能、呼吸功能、酸碱平衡,从而导致脑水肿以及神经细胞凋亡。
Abstract:
Objective To investigate correlative factors of brain injury and coagulation dysfunction in heat shock animal models induced by simulated military training. Methods 150 cases of SD rats were divided into heat treatment group and non-heat treatment group.The heat treatment group was 30,60,90,120 min and natural death group respectively,37 ℃ constant temperature processing without heat treatment group was 30,60,90,120 min and artificial execution groups,each group 15 rats.Compare the coagulation function (Automatic coagulation analyzer,Sysmex CS-5100) [the level of d-dimer,prothrombin time (PT),activated partial thromboplastin time (APTT),thrombin time (TT),and fibrinogen (FIB)],blood gas analysis[Blood oxygen pressure (PaO2),carbon dioxide pressure(PaCO2),blood pH],intracranial pressure,cerebral perfusion pressure and hippocampus apoptosis rate. Results The levels of D-dimer[(0.73±0.22),(0.93±0.34),(1.15±0.33)mg/L] in the heat-treated group were higher than those in the non-heat group at 90 min and 120 min (P<0.05).PT[(16.51±1.05),(19.79±1.97),(27.65±2.55),(36.28±3.88)s],APTT[(38.87±3.30),(49.95±7.88),(56.47±8.62),(63.84±9.94)s] and TT[(22.54±1.50),(28.74±3.93),(36.99±4.82),(40.18±6.26)s] of the heat-treated group were significantly prolonged (P<0.05) at 60,90,120 min and natural death compared with no heat-treated group.FIB levels of the heat-treated group at 60,90 min,120 min and the natural death[(2.01±0.18),(1.53±0.24),(1.05±0.22),(0.82±0.20)mg/dL] were significantly lower than those without heat treatment (P<0.05).There were significant differences in PaO2,PaCO2 and blood pH in the rats without heat treatment at different time points.The PaO2 and the blood pH showed a gradual decreasing trend and PaCO2 gradually increased (P<0.05).Brain water content[(74.25±0.5)%,(80.54±0.8)%,(83.54±0.2)%,(84.41±0.3)%] and hippocampus apoptosis rate[(19.52±0.61)%,(20.96±0.58)%,(24.33±1.61)%,(28.20±2.74)%] of the heat-treated group at 60,90,120 min and natural death were significantly higher than those without heat treatment (P<0.05).The heat-treated group intracranial pressure[(1.88±0.33),(2.19±0.11),(2.78±0.39),(3.21±0.58)kPa] were significantly higher than those without heat treatment at 90 min,120 min and natural death (P<0.05).Cerebral perfusion pressure[(8.25±0.71),(6.38±0.82),(5.59±0.69),(4.89±0.65)kPa] of heat-treated groups at 60,90 min,120 min and were significantly lower than those without heat treatment (P<0.05). Conclusion Long time military training in high temperature causes heat stroke,which is able to influence coagulation,respiratory function,and acid-base balance,brain edema and neuronal apoptosis.
