摘要:
目的探讨湿针针刺肌筋膜触发点结合牵伸训练在网球肘患者中的应用效果。方法选择2017年7月至2019年6月在上海市第二康复医院疼痛康复科就诊并明确临床诊断的网球肘患者84例，采用随机信封法分为结合组44例、对照组40例，结合组患者使用湿针针刺触发点结合牵伸训练的治疗方案，对照组患者仅予以冲击波疗法。分别在治疗前、治疗1个疗程后1周及治疗12周后观察比较2组“网球肘”评估（PRTEE）量表分值、疼痛自评量表（VAS）分值、握力指数、随访6个月复发及并发症发生情况。结果治疗12周后结合组的PRTEE分值、VAS分值和握力指数分别为[（13.22±1.46）分、（1.71±0.61）分和（85.32±7.31）分优于对照组[（22.31±2.63）分、（2.84±1.96）分和（77.03±6.49）分]，差异有统计学意义（P＜0.05 ）；随访6个月结合组疾病复发率2.27%，低于对照组的复发率15.00%（P＜0.05）。结论应用湿针针刺肌筋膜触发点结合牵伸训练治疗网球肘患者，能够改善网球肘患者的功能，缓解患者的疼痛、提高患者握力指数和降低复发率，具有较高的临床应用价值。
Abstract:
ObjectiveTo explore the effect of wet acupuncture myofascial trigger point combined with drafting training in patients with tennis elbow.MethodsA total of 84 patients with tennis elbow who were diagnosed clinically in the Department of Pain Rehabilitation of Shanghai Second Rehabilitation Hospital from July 2017 to June 2019 were randomly divided into two groups: 44 cases in the combined group and 40 cases in the observation group. The patients in the combined group were treated with wet acupuncture trigger point combined with stretch training, and the patients in the observation group were treated with shock wave therapy. The PRTEEscore of tennis elbow, VAS score of pain self-rating scale, grip strength index, recurrence and complications after 6 months follow-up were observed and compared between the two groups before treatment, 1 week after 1 course of treatment and 12 weeks after treatment.ResultsAfter 12 weeks of treatment, the PRTEE score, VAS score and grip strength index in the combination group were significantly higher than those in the observation group (22.31±2.63,2.84±1.96,77.03±6.49). After 6-month follow-up, the recurrence rate in the combination group (2.27%) was lower than that in the observation group (15.00%) (P<0.05).ConclusionThe application of wet acupuncture myofascial trigger point combined with stretch training in the treatment of tennis elbow patients can improve the function of tennis elbow patients, relieve pain, increase patients’ grip strength index and reduce recurrence rate. It has high clinical application value and is worth popularizing in clinical practice.
