摘要:
目的探讨无创胚胎染色体筛查技术(NICS)对军队不孕不育夫妇辅助生殖结局的影响。方法选择自2017年5月至2020年10月接受辅助生殖技术治疗的455对官兵夫妇进行前瞻性观察研究,其中133对实施卵母细胞胞浆内单精子注射(ICSI)方案,根据患者夫妇意愿进行分组,愿意接受NICS治疗45例,拒绝NICS治疗88例。88例中取消鲜胚移植后行单囊胚解冻复苏移植者26例,比较该26例(ICSI组)与NICS检测后首次单囊胚冻胚复苏移植39例(NICS组)的官兵夫妇取卵周期特征及复苏周期临床结局。结果NICS组患者男方年龄、女方年龄均较ICSI组高[男方年龄(35.79±4.52)岁 vs (31.69±3.10)岁,女方年龄(34.45±4.0)岁vs (29.65±2.04)岁,均P<0.001],NICS组较ICSI组抗苗勒管激素(AMH)[(3.39±2.59)ng/mL vs (5.23±3.27)ng/mL]、hCG日卵泡数(13.68±7.12 vs 17.77±8.88)、获卵数(12.37±6.72 vs 17.38±10.63)、成熟卵子数(10.95±6.21 vs 15.15±9.46)均显著降低(均P<0.05)。NICS组与ICSI组FET临床妊娠率相当(82.1% vs 80.8%)。结论军队不育夫妇实施ICSI治疗,胚胎植入前行NICS检测优选胚胎移植可改善临床结局。
Abstract:
ObjectiveTo explore the impact of non-invasive embryo chromosome screening (NICS) on the outcome of infertile couples in the military during assisted reproductive technology (ART) treatment.MethodsA prospective observational study of 455 couples of soldiers who received ART treatment from May 2017 to October 2020. Among them, 133 of them underwent intracytoplasmic sperm injection (ICSI) program due to the male or both factors, and grouped according to the wishes of the couple, 45 cases were willing to accept NICS testing, 88 cases refused NICS treatment. Among 88 cases, 26 cases select single blastocyst thawing and transplantation after fresh embryo transfer. Among 45 cases tested by NICS, 6 had no usable embryos, and the remaining 39 selected single embryo thawing and transplantation (NICS group). The cycle characteristics and clinical outcome of the resuscitation cycle werecompared between these 26 cases (ICSI group) and 39cases (NICS group).ResultsThe ages of the male and female in the NICS group were higher than those in the ICSI group (Male 35.79±4.52 vs 31.69±3.10,Female 34.45±4.0 vs 29.65±2.04, both P<0.001). The level of AMH (3.39±2.59 vs 5.23±3.27),number of follicles in hCG injection day (13.68±7.12 vs 17.77±8.88), number of eggs obtained (12.37±6.72 vs 17.38±10.63), and number of mature eggs (10.95±6.21 vs 15.15±9.46) in the NICS group were significantly lower than those of the ICSI group (both P<0.05). The clinical pregnancy rate of FET in the NICS group and the ICSI group was equivalent (82.1% vs 80.8%).ConclusionPre-implantation NICS detection and optimal euploid embryo transfer can improve the clinical outcome for infertile couples with ICSI treatment in the military.
