摘要:
目的比较切开复位双排可吸收锚钉桥式缝合与空心螺钉固定治疗后交叉韧带胫骨止点撕脱性骨折的临床疗效 。方法回顾性分析2016年5月-2019年5月解放军联勤保障部队第九○一医院收治的急性后交叉韧带胫骨止点撕脱性骨 折35例患者的临床资料,均实施切开复位固定手术,根据固定方式不同分为2组:双排锚钉组15例采用双排可吸收锚 钉桥式缝合固定,空心螺钉组20例采用空心螺钉固定的方法进行治疗。统计并比较2组的手术时间、切口长度、术后 下地时间、住院时间、住院费用、骨折愈合时间、末次随访Lysholm评分及术后并发症等情况。结果2组患者均顺利 手术,均无切口相关并发症。双排锚钉组手术时间[(77.33±9.88)min]明显长于空心螺钉组[(59.75±7.86) min],住院费用[(28 132±2096)元]明显高于空心螺钉组[(15 904±1113)元],术后下地时间[(2.07±0.70) d]明显小于空心螺钉组[(3.80±1.64)d],差异均具有统计学意义(P<0.05)。双排锚钉组15例均获得解剖复位 ,空心螺钉组19例获得解剖复位。2组切口长度、住院时间、骨折愈合时间、末次随访Lysholm评分比较差异无统计 学意义(P>0.05)。结论切开复位双排可吸收锚钉桥式缝合与空心螺钉固定治疗后交叉韧带胫骨止点撕脱性骨折均 可获得良好的手术效果。双排可吸收锚钉桥式缝合手术固定牢固,下地时间更短,但手术时间更长,住院费用更高 。
Abstract:
ObjectiveTo compare the outcomes of open reduction through double row absorbable anchor bridge suture and hollow nails for acute posterior cruciate ligament (PCL) avulsion fracture.MethodsFrom May 2015 to May 2019, 35 patients with acute PCL avulsion fractures were treated by open reduction through double row absorbable anchor bridge suture and hollow nails. There were 15 cases in the double row absorbable anchor group and 20 cases in the hollow nails group. The operation time, cut length, postoperative ambulation time, hospitalization expenses, healing time of fracture and Lysholm score of patients at the last follow-up were compared between the two groups.ResultsAll the patients in both groups had operation performed smoothly without serious complication. The operation time in double row absorbable anchor group was (77.33±9.88) min which was longer than the hollow nails group (59.75±7.86) min, P<0.05). The hospitalization expenses in the double row absorbable anchor group was (28 132±2096) yuan which was higher than the hollow nails group (15 904±1113) yuan, P<0.05). The postoperative ambulation time in the double row absorbable anchor group was (2.07±0.70) d which was shorter than the hollow nails group ((3.80±1.64) d, P<0.05). Anatomical reduction was obtained in 15 cases in the double row absorbable anchor group and 19 cases in the hollow nails group. There were no significant difference between the two groups in cut length, healing time of fracture and Lysholm score of patients at the last follow-up (P>0.05).ConclusionBoth open reduction through double row absorbable anchor bridge suture and hollow nails for acute PCL avulsion fracture have good clinical outcome. Double row absorbable anchor bridge suture has advantage of reliable fixation and shorter postoperative ambulation time cut length, and hollow nails fixation has advantages of less operation time and higher hospitalization expenses.
