摘要:
目的探讨磁牵引辅助下经脐自然腔道软式内镜（MAG-E-NOTES）切除胆囊的可行性及安全性。方法采用8只比格犬，用经脐自然腔道软式内镜手术的方式，在磁牵引辅助下切除胆囊，记录手术时间、有无出血、胆瘘、穿孔、肝损伤、腹腔器官损伤等并发症情况。结果8只比格犬均顺利完成磁牵引辅助下经脐自然腔道软式内镜胆囊切除术，平均手术时间为(97.38±23.79)min，除1只比格犬出现胆瘘外，其余比格犬在术中均无大出血、穿孔、胆瘘、腹部器官损伤等情况发生，3个月后均健康存活。结论 牵引辅助下经脐自然腔道软式内镜胆囊切除术（E-NOTES）胆囊切除术是安全、可行的，值得进一步研究为临床提供更多胆囊切除术的手术方式。
Abstract:
ObjectiveTo explore the feasibility and safety of cholecystectomythrough natural orifice transluminal endoscopic surgery by magnetic anchoring and guidance system.Methods8 Beagle dogs were used in this study and completed cholecystectomy by natural orifice transluminal endoscopic surgery(NOTES). Recoring the operation time, whether a massive hemorrhage, biliary fistula, liver damage, abdominal organ damage or not and other complications.ResultsAll the 8 Beagle dogs have completed cholecystectomy by natural orifice transluminal endoscopic surgery successfully.The mean operative time was（97.38±23.79）min. The operation was expected to be completed successfully in all cases. There were no adverse reactions such as massive hemorrhage, bile leakage, liver damage, abdominal organ damage and other complications except bile leakage occured in one Beagle dog.ConclusionCholecystectomy through natural orifice transluminal endoscopic surgery by magnetic anchoring and guidance system is safe and feasible, which is worthy of further study and providing clinical value for cholecystectomy.
