摘要:
目的对比分析基于人体测量学指标和体型指标选择臀部肌注部位与传统定位方法注射后的疼痛严重程度，为临床肌内注射的部位选择提供参考依据。方法采用随机对照试验，共纳入162名来我院接受甲钴胺臀部肌肉注射的患者。对照组按照传统定位方法接受臀大肌注射；测量学指标组根据BMI、腰围和髂前上棘到髂结节距离决定注射部位；体型指标组根据体型、BMI和性别决定注射部位；注射后通过VAS进行疼痛评估。结果受试者平均年龄为（40.13±11.26）岁，其中男性占46.67% (n=77)。多元线性回归分析显示，与对照组相比，体型指标组注射后疼痛显著较低（r2：0.28；回归系数=-0.43；95%CI：-0.83，-0.05；P=0.048）。注射后左侧疼痛程度较右侧严重（r2：0.26；回归系数=0.46；95%CI：0.07, 0.88；P=0.024）。结论基于体型选择臀部注射部位对缓解注射疼痛具有显著影响，并且在优势侧进行肌内注射可减轻疼痛，具有一定的临床应用价值。
