摘要:
目的探讨心脏束室旁道患者的临床特点、心电图特征、腔内电生理诊断及治疗。方法报道2例心脏束室旁道并复习相关文献。结果2例患者均显现出典型束室旁道特点,心电图主要表现为PR间期正常或轻度延长、有轻度预激波、QRS宽度多在正常范围。腔内电生理特征表现为旁道仅有前传功能,伴递减性传导,心房递增起搏时,AH间期逐渐延长,而HV间期始终不变,但较正常的HV间期短,且体表心电图QRS波形也不变。同时给予心房、心室超速和程序刺激均未诱发心动过速,静滴异丙肾上腺素没有诱发心律失常,未行射频消融术。结论心脏束室旁道具有特征性的腔内电生理特点,大多不参与心动过速,应避免消融,减少希浦系统损伤的几率。通过体表心电图很难与其他旁道相鉴别,虽然静脉推注三磷酸腺苷(ATP)是有效的无创方法,但仍有漏诊、误诊的可能。
