摘要:
目的观察行超声引导下多点穿刺切割囊壁后挤压并注射复方倍他米松治疗基层官兵腕部腱鞘囊肿的临床治疗效果。方法回顾性分析2019年1月至2021年9月在陆军第七十一集团军医院门诊行超声引导微创治疗腕部腱鞘囊肿的基层官兵11例的临床资料，均在超声引导下行囊肿壁多点穿刺切割并以挤压放松再挤压的手法排尽囊液，后注入2％利多卡因注射液和复方倍他米松注射液1∶1配比的1 mL混悬液，弹力绷带加压包扎。统计患者性别、年龄、囊肿大小、部位、术中及术后并发症等。比较术前及术后最后一次随访时囊肿大小变化和疼痛视觉模拟评分法（VAS）变化。结果11例均一次性穿刺挤压引流成功，术中2例出现晕针现象。术后11例全部随访成功，其中1例治疗3个月后复发，2例治疗5个月后复发，均为惯用手侧，总体有效率为72.7％(8/11)。术前与术后VAS评分比较差异有统计学意义（2.90±1.14 vs 1.04±0.28，P<0.01）。结论 超声引导下穿刺抽吸联合挤压引流并注射复方倍他米松治疗腱鞘囊肿操作简便、安全性高、复发率低，能够有效缓解手腕部疼痛不适症状，是部分罹患腱鞘囊肿的基层官兵的首选治疗方式。
