摘要:
目的息肉样子宫内膜异位症（PEM）是子宫内膜异位症（EMs）的一种罕见病变类型，易误诊。文中探讨PEM的临床特征及诊断方法。方法回顾性分析2017年7月-2020年12月于福建省妇幼保健院收治的7例PEM患者的临床资料，总结其临床特征、诊疗及预后情况。结果患者发病年龄31～41（38.00±1.72）岁。6例盆腔重度黏连，5例合并有子宫腺肌病和异常子宫出血，4例合并有子宫内膜息肉及不孕症，临床表现多以盆腔包块、压迫症状体征、痛经、出血为主。CT和（或）MRI误诊4例，其中3例误诊为恶性肿瘤。6例CA125抗原明显升高[103.8~762.6（225.47±117.49）U/mL]，另1例为15.3 U/mL且无特异临床表现。7例均在术后病理确诊，病理大体特征为囊肿、内或外生性息肉样肿块；镜下均由子宫内膜样腺体及间质成分构成，比普通EMs更具多样性。结论PEM多合并子宫腺肌病、子宫内膜息肉、不孕症，病理学为诊断金标准，CA125抗原可辅助诊断。
