摘要:
目的 探讨急性呼吸窘迫综合征(acute respiratory distress syndrome，ARDS)患者机械通气时血清N末端B型钠尿肽前体(NT-proBNP)水平与潮气量相关性。方法 ARDS患者84例、非ARDS重症患者40例(对照组)，合并随机分为低潮气量组(n=64)和正常潮气量组(n=60)。在维持气道压平稳的前提下，低潮气量组患者潮气量设定为6 mL/kg，正常潮气量组设定为10 mL/kg。记录呼吸机相关参数等，并测定血清NT-proBNP水平。应用Pearson线性相关分析NT-proBNP和潮气量的相关性。结果 低潮气量组和正常潮气量组ARDS患者通气时的血清NT-proBNP水平均高于对照组患者(P<0.05)。低潮气量组ARDS患者机械通气6 d后的血清NT-proBNP水平低于正常潮气量组ARDS患者(P<0.05)。血清NT-proBNP水平在机械通气时与液体平衡呈正相关(P<0.05)，但与潮气量无明显相关性(P>0.05)。结论 ARDS患者低潮气量组在不同时间点NT-proBNP水平均低于正常潮气量组，但与潮气量无相关性。
Abstract:
Objective To investigate the relation between NT-proBNP and tidal volume in ARDS patients with mechanical ventilation. Methods Eighty-four ARDS and forty non-ARDS patients were enrolled and randomly divided into two groups: low tidal volume group and normal tidal volume group. Under the condition of normal airway pressure, patients in the low tidal volume group were received with a 6 mL/kg tidal volume mechanical ventilation while patients in normal tidal volume group were received 10 mL/kg tidal volume. The parameters of mechanical ventilation and levels of NT-proBNP in different time points were recorded. Pearson analysis was used to evaluate the relation between NT-proBNP and tidal volume. Results The levels of NT-proBNP of ARDS patients in low tidal volume group were lower than those of non-ARDS patients in low tidal volume group(P<0.05), which could also be observed in normal tidal volume group. The levels of NT-proBNP of ARDS patients in low tidal volume group were lower than those of ARDS patients in low tidal volume group in 6 days after mechanical ventilation(P<0.05). The levels of NT-proBNP showed a positive relationship with fluid balance(P<0.05) but tidal volume(P>0.05). Conclusion The levels of NT-proBNP of ARDS patients in low tidal volume group were lower than those in normal tidal volume group in different time points. However, it showed no relation between NT-proBNP and tidal volume.
