摘要:
目的 探讨高龄2型糖尿病合并缺血性脑血管病患者的临床特征。方法 选取某院2013年1月-2014年1月高龄2型糖尿病合并缺血性脑血管病患者50例为观察组，同期单 纯缺血性脑血管高龄患者50例为对照组，检测两组的各项血脂水平，观察组予以胰岛素、适当运动及合理控制饮食控制血糖后，两组均予以阿司匹林与奥扎格雷钠治疗，两组均治 疗2周，比较两组的各项血生化指标水平及临床有效率、神经功能缺损程度。结果 观察组的尿酸（UA）、纤维蛋白原（FIB）、三酰甘油（TG）、血胆固醇（TC）水平显著高于对 照组（P<0.05），而高密度脂蛋白（HDL）显著低于对照组(P<0.05)；观察组、对照组的临床有效率分别为88.0%、58.0%，差异具有统计学意义（P<0.05）；两组治疗后的神经功能 缺损评分情况均有显著改善，对照组神经功能缺损的评分显著优于观察组（P<0.05）。结论 2型糖尿病合并缺血性脑血管病患者常合并FIB、UA及血脂代谢异常，治疗效果较差， 应积极控制血糖、血脂水平。
Abstract:
Objective To investigate the clinical characteristics of elderly type 2 diabetes mellitus patients with ischemic cerebrovascular disease. Methods Selected 50 cases of elderly type 2 diabetic patients with ischemic cerebral vascular disease as the observation group in our hospital from January 2013 to January 2014， the 50 cases of patients with selected purely ischemic cerebral vascular as control group. The level of blood lipid indexes were detected in two groups. The observation group were treated with insulin， proper exercise and reasonable diet to control blood glucose. Two groups were treated with aspirin and sodium ozagrel for 2 weeks. Compare blood biochemical indexes， clinical comparison of efficiency and degree of nervous functional defect in two groups. Result Uric acid (UA)， fibrinogen (FIB)， triglyceride (TG)， serum cholesterol (TC) levels of the observation group were significantly higher than that in the control group (P<0.05). High density lipoprotein (HDL) were significantly lower than that of control group (P<0.05). The effective rate of the observation group and the control group were 88.0%， 58.0% respectively with difference statistically significant (P<0.05). After treatment， theneural function of the two groups defect score were significantly improved， of nerve function defect score in the observation group was significantly lower than that in the control group (P<0.05). Conclusion Type 2 diabetes mellitus complicated with ischemic cerebral vascular disease patients often associated with FIB， UA and dyslipidemia and the treatment result is bad. It should actively control blood sugar， blood lipid level.
