摘要:
目的 探讨加速康复外科 (ERAS) 理念对单孔腹腔镜下子宫肌瘤剔除术患者围术期的效果。 方法 回顾性分析2017年9月至2018年12月上海市第一人民医院宝山分院收治的80例单孔腹腔镜下行子宫肌瘤剔除术患者的临床资料,根据围术期不同的处理方法,分为加速康复外科组(ERAS组)和传统处理组(对照组),比较2组患者手术时间、手术中出血量、实验室指标(白细胞、中性粒细胞百分比、CRP、K+、Na+、Cl-)、术后恶心呕吐的发生、术后排气时间、住院时间以及住院费用等。 结果 ERAS组手术时间、术中出血量、白细胞、Na+、Cl-均低于对照组,但差异均无统计学意义(P>0.05);ERAS组中性粒细胞百分比、CRP低于对照组(76.6% vs 82.3%,28.7 mg/L vs 43.5 mg/L),K+高于对照组(3.9 mmol/L vs 3.3 mmol/L),差异均有统计学意义(P<0.05);ERAS组较对照组术后恶心呕吐发生率(10% vs 32.5%)、术后排气时间[(13.6±3.8)h vs (17.6±3.9)h]、住院时间[(4.2±0.4)d vs (5.3±0.6)d]及住院费用[(9752.8±175.5)元 vs (9936.4±220.3)元]均显著降低,差异均有统计学意义(P<0.05)。 结论 加速康复外科理念应用于单孔腹腔镜下子宫肌瘤剔除术患者的围术期,可以取得较好的临床效果,缩短患者住院时间,降低住院费用,值得临床推广和使用。
Abstract:
Objective To investigate the efficacy of perioperative enhanced recovery after surgery (ERAS) on patients undergoing single-hole laparoscopic myomectomy. Methods Eighty patients who had been undergoing single-hole laparoscopic myomectomy due to uterine myomectomy in Shanghai First People’s Hospital Baoshan Branch during september 2017 to december 2018 were divided into enhanced recovery after surgery (ERAS) and conventional group (control group). The operative time, intraoperative blood loss, laboratory parameters (leukocyte, neutrophil percentage, CRP, K+, Na+, Cl-), the postoperative nausea vomiting, postoperative anal exhaust time, postoperative hospital stay, hospitalization costs, etc were compared. Results The operation time, intraoperative blood loss, the levels of leukocytes, Na+ and Cl- in ERAS group were lower than those in control group, but there was no statistically significant (P>0.05). The percentage of neutrophils, CRP in ERAS group were lower than those in control group (76.6% vs 82.3%, 28.7 mg/L vs 43.5 mg/L, P<0.05). K+ in ERAS group was higher than that in control group(3.9 mmol/L vs 3.3 mmol/L, P<0.05). The incidence of nausea and vomiting after operation(10% vs 32.5%), the time of exhaust after operation[(13.6±3.8)h vs (17.6±3.9)h], postoperative hospital stay[(4.2±0.4)d vs (5.3±0.6)d] and hospitalization costs [(9752.8±175.5)yuan vs (9936.4±220.3)yuan] in ERAS group were statistically significant different with that in control group (P<0.05). Conclusion The efficacy of perioperative enhanced recovery after surgery concept on patients undergoing single-hole laparoscopic myomectomy has a better therapeutic effect, shortens the hospitalization time, reduces the cost of hospitalization. It is worth clinical promotion and application.
