目的 通过与常规吸氧比较,评价魏氏鼻咽通气道联合声门上喷射通气在无痛胃镜检查中的安全性和可行性。 方法 选择2019年3月1日至3月25日解放军联勤保障部队第九一○医院ASA I~Ⅲ级、18岁≤年龄<65岁的行无痛胃镜检查患者100例,采用完全随机法将患者分为喷射通气组和常规吸氧组,每组50例。记录2组患者低氧血症的发生情况和解除低氧血症的办法,记录丙泊酚总量和检查时间以及其他不良事件。 结果 常规吸氧组有8例出现低氧血症患者(呼吸抑制6例,轻度低氧血症2例);其中6例通过托下颌,2例通过面罩通气来纠正低氧血症,而喷射通气组未出现低氧血症,组间差异有统计学意义(P<0.05)。常规吸氧组胃镜操作时间明显长于喷射通气组[(6.9±2.8 min) vs (6.0±1.3)min,P<0.05]。2组丙泊酚用量及不良事件发生率比较差异无统计学意义(P>0.05)。 结论 魏氏鼻咽通气道联合声门上喷射通气应用于在无痛胃镜检查能够减少低氧血症的发生,缩短检查时间,安全可行。

Objective To evaluate the efficacy and safety of WEI Nasal Jet Tube with supraglottic pulsatile jet oxygenation for painless gastroscopy by comparing with oxygen. Methods 100 patients of ASA I-Ⅲ (18 years≤aged<65 years,undergoing for painless gastroscopy) were randomly divided into 2 groups:jet group received WEI Nasal Jet Tube with supraglottic pulsatile jet oxygenation and normal group received oxygen. The hypoxemia and processing method of hypoxemia were recorded. The total of propofol,the time of gastroscopy and adverse events were also recorded. Results 8 patients occurred hypoxemia (6 patients subclinical respiratory depression,2 patients mild hypoxemia),6 of whom required jaw thrust and 2 of whom required face mask ventilation to correct the hypoxia in the normal group. However, no patient occurred hypoxemia in the jet group,which was significantly more in the normal group than that in the jet group (P<0.05). The time of gastroscopy was longer in the normal group than that in the jet group [(6.9±2.8 min) vs (6.0±1.3)min,P<0.05]. There was no significant difference between the dosage of propofol and the incidence of adverse events in two groups(P>0.05). Conclusion WEI nasal jet tube with supraglottic pulsatile jet oxygenation for painless gastroscopy can reduce the incidence of hypoxemia and shorten the time of gastroscopy,and is safe and feasible.
