摘要:
目的 探讨2型糖尿病患者心外膜脂肪厚度(EFT)与颈动脉内膜中层厚度(CIMT)的关系。 方法 选取2018年1月至2019年7月在解放军联勤保障部队第九○四医院就诊的38例2型糖尿病患者作为糖尿病组,另同时期招募30例年龄和性别匹配的健康对照者作为对照组。所有研究对象均接受EFT和CIMT的超声检查,比较2组研究对象的一般特征,采用双变量相关分析评估EFT、CIMT和其他变量之间的相关性。 结果 糖尿病组患者EFT、CIMT[(6.2±1.3)mm、(0.8±0.1)mm]均高于对照组[(4.6±1.0)mm、(0.5±0.1)mm],差异均有统计学意义(P<0.01)。在2型糖尿病患者中,EFT与CIMT呈显著正相关(r=0.573,P<0.001)。EFT与体重指数、病程、空腹血糖、糖化血红蛋白(HbAlc)、胰岛素抵抗指数、总胆固醇、低密度脂蛋白和收缩压显著相关(P<0.05)。逐步回归分析显示,CIMT、病程、三酰甘油和体重指数是EFT独立预测因子,CIMT是最重要的预测因子(β=0.384,t=4.059,P<0.001)。ROC曲线分析显示,EFT危险截点值为6.1 mm,敏感性和特异性分别为71.4%和72.3%。 结论 2型糖尿病患者CIMT和EFT较厚,EFT是亚临床动脉粥样硬化的强预测因子。
