摘要:
目的制订基于渐进模型的适合老年髋关节置换患者的运动处方,对老年患者进行术后处方化的康复训练,探讨其临床效果。方法选取60例接受髋关节置换术的老年患者,按照随机数字表法纳入对照组和处方组,对照组予以常规护理和肌力训练,处方组予以渐进模型的运动处方化进行管理。对2组患者在术后1个月末、3个月末进行起立-走时间测定和髋关节功能Harris评分比较;在术前和术后进行Berg平衡量表评分。结果术后1个月末的起立-行走时间处方组为(35.01±3.70)s、对照组为(36.94±3.26)s;在3个月末,处方组为(32.15±3.04)s、对照组为(34.44±2.83)s;处方组较对照组时间缩短,差异有统计学意义(t=-2.147,P<0.05;t=-3.015,P<0.05)。术后1个月末,处方组Harris评分为(48.83±6.56)分、对照组为(42.47±7.49)分;在3个月末,处方组为(77.03±6.78)分、对照组为(66.60±8.60)分;处方组1个月末和3个月末Harris评分均优于对照组,差异有统计学意义(P<0.01)。术后Berg平衡量表评分处方组为(43.90±4.15)分、对照组为(41.73±2.78)分,处方组较对照组高,差异有统计学意义(P<0.05)。结论基于渐进模型的运动处方管理方案能够能更好地促进老年髋关节置换患者的姿势变换、步行能力和平衡功能等的恢复。
Abstract:
ObjectiveExercise prescription for elderly patients with hip arthroplasty was formulated based on a progressive model, and prescription rehabilitation training was conducted for elderly patients after total hip arthroplasty or artificial femoral head arthroplasty, and its effect was discussed.MethodsSixty elderly patients receiving hip arthroplasty were selected and included into the control group and the observation group according to a random number table method. The control group was given routine nursing and muscle strength training guidance, and the observation group was administered with an exercise prescription based on the progressive model.At the end of the first month and the end of the third month after operation, the stand-up time and walking time were measured and the Harris score of hip joint function was compared between the two groups.The Berg Balance Scale was scored preoperatively and postoperatively.ResultsThe time of standing and walking at the end of the first month after operation was (35.01±3.70)s in the observation group and (36.94±3.26)s in the control group.At the end of 3 months, the time in the observation group was (32.15±3.04)s and that in the control group was (34.44±2.83)s. The time in the observation group was shorter than that in the control group, and the difference was statistically significant (t=-2.147, P<0.05; t=3.015, P<0.05). Harris score at the end of the first month after surgery was (48.83±6.56) in the observation group and (42.47±7.49) in the control group.At the end of the third month, it was (77.03±6.78) in the observation group and (66.60±8.60) in control group. Harris score of the observation group at the end of the first and the end of the third month was better than those of the control group, and the difference was statistically significant (P<0.05). Postoperative Berg Balance Scale score was (43.90±4.15) in the observation group and (41.73±2.78) in the control group, which was higher in the observation group than the control group, and the difference was statistically significant (P<0.05).ConclusionThe exercise prescription management scheme based on progressive model can better promote the recovery of posture change, walking ability and balance function in elderly patients with hip replacement.
