摘要:
目的探讨以江苏省为主的区域性女性生殖道疾病组织中人乳头瘤病毒(HPV)感染型别的分布情况及其临床意义。方法从患者的石蜡组织中提取HPV DNA,采用荧光定量基因扩增技术,对女性248例外阴尖锐湿疣,144例宫颈尖锐湿疣,281例宫颈上皮内瘤变(CIN)1级病变、172例CIN2级病变、209例CIN3级病变,456例宫颈鳞癌组织标本进行26种HPV分型检测,并对HPV型别分布状况进行对比分析。结果248例外阴尖锐湿疣患者中,HPV总感染率为89.92%(223/248),低危型HPV感染率为60.48%(150/248);144例宫颈尖锐湿疣患者中,HPV总感染率为93.06%(134/144),低危型HPV感染率为66.67%(96/144);281例CIN1级病变患者中,HPV总感染率为49.47%(139/281),高危型HPV感染率为40.93%(115/281);172例CIN2级病变患者中,HPV总感染率为83.14%(143/172),高危型HPV感染率为72.09%(124/172);209例CIN3级病变患者中,HPV总感染率为93.30%(195/209),高危型HPV感染率为81.34%(170/209);456例宫颈鳞癌患者中,HPV总感染率为94.08%(429/456),高危型HPV感染率为89.25%(407/456)。结论外阴和宫颈尖锐湿疣患者以低危型HPV感染为主,CIN1级、CIN2级和CIN3级病变患者以高危型HPV感染为主,而宫颈鳞癌患者以高危型HPV感染占绝对优势。随着女性生殖道病变程度的增加,高危型HPV感染率也随之增加,监控高危型HPV,是宫颈癌防治的重要环节。HPV分型检测有利于病理科医师对女性生殖道尖锐湿疣的诊断,也有利于妇科医师对高危型HPV感染者,尤其是高风险HPV感染者的随访。
Abstract:
ObjectiveTo investigate the distribution of human papilloma virus (HPV) types and its clinical significance in regional female reproductive tract disease tissues mainly in Jiangsu Province.MethodsHPV DNA was extracted from paraffin tissues of female reproductive tract diseases, and fluorescent quantitative gene amplification technology was used to detect 26 kinds of HPV types from 248 cases of vulvar condyloma acuminatum (CA), 144 cases of cervical CA , 281 cases of CIN1 lesions, 172 cases of CIN2 lesions, 209 cases of CIN3 lesions, and 456 cases of cervical squamous cell carcinoma tissue samples, and the distribution of HPV types in various female reproductive tract disease was compared and analyzed.ResultsThe total HPV infection rate was 89.92%(223/248), which the low-risk HPV infection rate was 60.48%(150/248)in 248 cases of vulvar CA; The total HPV infection rate was 93.06%(134/144), which the low-risk HPV infection rate was 66.67%(96/144)in 144 cases of cervical CA; The total HPV infection rate was 49.47%(139/281), which the high-risk HPV infection rate was 40.93%(115/281)in 281 cases of CIN1 lesions; The total HPV infection rate was 83.14%(143/172), which the high-risk HPV infection rate was 72.09%(124/172)in 172 cases of CIN2 lesions; The total HPV infection rate was 93.30%(195/209), which the high-risk HPV infection rate was 81.34%(170/209)in 209 cases of CIN3 lesions; The total HPV infection rate was 94.08%(429/456), which the high-risk HPV infection rate was 89.25%(407/456)in 456 cases of cervical squamous cell carcinoma.ConclusionPatients with vulvar CA and cervical CA are mainly low-risk HPV infections, CIN1 andCIN2 and CIN3 disease patients are mainly high-risk HPV infections, while cervical squamous cell carcinoma patients are predominantly high-risk HPV infections. With the increase in degree of diseases in the female genital tract, the infection rate of high-risk HPV also increases. Monitoring the principal carcinogenic high-risk HPV is an important link in the prevention and treatment of cervical squamous cell carcinoma. HPV typing detection is beneficial to pathologists in the diagnosis of female genital tract wart lesions, to gynecologists in the diagnosis and treatment of female genital tract HPV infectious diseases, and it is also beneficial to gynecologists in the follow-up of patients with high-risk HPV infections.
