摘要:
目的 探讨丙泊酚联合小剂量喷他佐辛用于无痛人工流产术的可行性及其优点。 方法 160例ASAⅠ级早期妊娠患者随机分为四组，每组40例。 A组:丙泊酚2.5 mg/kg； B组:芬太尼0.001 mg/kg、丙泊酚2 mg/kg；C组:喷他佐辛0.3 mg/kg、丙泊酚2 mg/kg；D组:喷他佐辛0.6 mg/kg、丙泊酚2 mg/kg。术中出现肢体扭动者酌情追加丙泊酚30~40 mg。观察比较麻醉前后呼吸循环变化、麻醉镇痛效果、术后恢复情况及不良反应等。结果 四组患者都成功完成手术。各组用药后均有明显循环抑制(P<0.05)， 以A组最为显著(P<0.01)。呼吸抑制发生率B组显著高于其他组(P<0.01)。术中镇痛不全和术后腹痛发生率A组显著高于其他各组(P<0.05)；苏醒及离院时间D组显著长于A、B、C组(P<0.01)。结论 丙泊酚联合小剂量喷他佐辛(0.3 mg/kg)能较好地满足无痛人工流产手术麻醉需要，并能减少单用丙泊酚时镇痛不全、严重循环抑制和加用芬太尼时明显的呼吸抑制，值得推广应用于各类无痛诊疗技术。
Abstract:
Objective To explore the feasibility of propofol in combination with small doses of pentazocine in painless abortion. Methods 160 early-pregnant women with ASAⅠwere randomly divided into four groups (40 patients each group). Patients in group A were given intravenous propofol; Patients in group B were given fentanyl 0.001 mg/kg followed by propofol intravenously; Patients in group C and D were given 0.3 mg/kg or 0.6 mg/kg pentazocine respectively before intravenous propofol. The initial dose of propofol in group A was 2.5 mg/kg and 2 mg/kg in other groups, and additional doses will be applied when body motion occurred. The changes of respiratory and circulatory function, anesthesia and analgesia effects, postoperative recovery profile, and side-effects were observed. Results The operations were successful in all the patients. All patients showed circulatory depression after anesthesia, and the patients in group A were the severest (P<0.05). Among all patients in four groups, the respiratory depression incidence rates of patients in group B was the highest (P<0.01), the intraoperative insufficient analgesia and postoperative abdominal pain incidence rates of patients in group A was the most significant (P<0.05). In addition, the awake time and departure time of patients in group D was the longest (P<0.05). Conclusion Propofol in combination with small doses of pentazocine (0.3 mg/kg) could decrease the incidence of insufficient analgesia compared with using propofol alone and alleviate fentanyl induced respiratory depression in painless artificial abortion, which is the suitable anesthetic compatibility.
