摘要:
目的探讨肝素钠肌醇烟酸酯乳膏对白兔皮肤冻伤的治疗作用,及肝素钠肌醇烟酸酯乳膏多次皮肤给药对兔正常和破损皮肤的刺激反应。方法健康兔36只,按照体重排序,按随机数字表法随机分成6组,包括:空白对照组,模型对照组,肝素钠肌醇烟酸酯乳膏低、中、高剂量组(0.14 g/kg、0.28 g/kg、0.56 g/kg),肝素钠乳膏组(0.14 g/kg),每组6只。除空白对照组外,其余各组制备兔皮肤冻伤模型:将直径3 cm的纱布团沾满液氮(成饱和程度),将其贴紧兔背部实验部位,造成一个直径3 cm的皮肤冻伤模型。肝素钠肌醇烟酸酯乳膏低、中、高剂量组白兔涂抹相应浓度的肝素钠肌醇烟酸酯乳膏,观察兔皮肤表观变化、肿胀程度、存活面积和冻区病理学检查等指标,考察肝素钠肌醇烟酸酯乳膏白兔皮肤冻伤模型的治疗作用。采用同体左右自身对比,将肝素钠肌醇烟酸酯乳膏多次涂抹于正常皮肤及破损皮肤后进行皮肤刺激试验。结果肝素钠肌醇烟酸酯乳膏低、中、高剂量组能够使冻伤后第14天白兔冻伤面颜色趋于正常,毛发生长趋于正常,结痂逐渐愈合,皮肤愈合率分别为(67.45±8.73)%、(65.65±8.18)%、(81.66±3.66)%。肝素钠肌醇烟酸酯乳膏中剂量组、高剂量组与模型对照组比较,可分别使红细胞压积、红细胞聚集指数、单核细胞比率和嗜酸性粒细胞比率明显升高。肝素钠肌醇烟酸酯乳膏能明显降低冻伤初期局部组织溃疡面积和肉芽组织面积,能明显降低冻伤初期局部组织炎细胞程度。白兔正常皮肤多次接触肝素钠肌醇烟酸酯乳膏低、中、高剂量后不同时间均未见异常反应,白兔破损皮肤多次接触肝素钠肌醇烟酸酯乳膏低、中剂量无刺激性,高剂量(0.28 g/kg)有轻度刺激反应(0~2.75分)。结论肝素钠肌醇烟酸酯乳膏对白兔皮肤冻伤具有良好的治疗作用,且对正常皮肤局部用药安全,对破损皮肤高剂量局部用药有轻度刺激作用,应酌情控制用量。
Abstract:
ObjectiveTo explore the therapeutic effect of heparin sodium inositol nicotinate cream for mild frostbite model on rabbit skin, and the Irritation and Allergy Test of Repeated skin administration of heparin sodium inositol nicotinate cream on normal and broken skin of rabbits.MethodsThirty-six healthy white rabbits were randomly divided into blank control group, model controlgroup, the low-dose, medium-dose and high-dose heparin sodium inositol nicotinate cream group (0.14 g/kg, 0.28 g/kg, 0.56 g/kg) and heparin sodium cream group (0.14 g/kg), with 6 rabbits in each group. Except the blank control group, rabbit skin frostbite models were prepared in other groups:A gauze ball with a diameter of 3 cm was soaked with liquid nitrogen (to a degree of saturation), and it was pressed tightly on the rabbit in the experimental group to cause a skin mild frostbite model with a diameter of 3 cm. Rabbits in each group were applied with low-dose, medium-dose and high-dose of heparin sodium inositol nicotinate cream to observe the appearance changes, swelling degree, survival area and pathological examination of the frozen area of rabbits as the observation index. Using the left and right self-comparison of the same body, the heparin sodium inositol nicotinate cream was applied to normal skin and damaged skin several times and then the skin irritation test was performed.ResultsOn the 14th day after frostbite, the color of frostbite surface, hair growth and scab gradually healed in the low-dose, medium-dose and high-dose heparin sodium inositol nicotinate cream groups were normal, and the skin healing rates were(67.45±8.73)%, (65.65±8.18)% and (81.66±3.66)%, respectively. Compared with model group, the hematocrit, erythrocyte aggregation index, monocyte ratio and eosinophil ratio were significantly increased in medium dose group and high dose heparin sodium inositol nicotinate cream group, respectively. The heparin sodium inositol nicotinate cream significantly reduced the area of local tissue ulcer and granulation tissue in the early stage of frostbite and significantly reduced the degree of inflammatory cells in local tissues at the initial stage of frostbite. The rabbit normal skin group was exposed to heparin sodium inositol nicotinate cream for many times and no abnormal reaction was observed at different times. In the damaged skin group of white rabbits, there was no irritation after repeated exposure to heparin sodium inositol niacin cream at low and medium doses, but mild irritation after exposure to heparin sodium inositol niacin cream at high doses (0.28 g/kg).ConclusionHeparin sodium inositol nicotinate cream has a good effect on the treatment of white rabbit skin frostbite, and it is safe for normal skin topical application, and has a mild stimulative effect on damaged skin with high dose topical application, so the dosage should be controlled as appropriate.
