摘要:
目的 探讨结肠癌患者术前红细胞分布宽度（RDW）与患者术后存活率的相关性。 方法 回顾性分析2011年1月至2013年12月于南京医科大学附属淮安第一医院接受外科手术治疗的结肠癌119例患者的临床资料，并随机选取同期住院诊治的溃疡性结肠炎（UC）119例作为对照，比较结肠癌患者和UC患者外周血RDW的差异。同时以结肠癌患者术前的RDW均值为界将其分为高RDW组和低RDW组，对其术后进行长期随访，分析RDW与术后存活率之间的相关性。 结果 结肠癌患者的RDW高于UC患者[（43.48±4.06）% vs (41.76±4.93）%，P<0.01]。结肠癌患者术前RDW与癌胚抗原（CEA）呈显著正相关（r=0.748，P<0.01）。RDW与肿瘤位置、肿瘤大小、病理分级、肿瘤浸润深度、术后病理淋巴结是否阳性均无关（P>0.05），且与患者术后1年的存活率无关（P>0.05）。高RDW组在术后3年和5年的存活率明显低于低RDW组（68.6% vs 85.3%，40.0% vs 75.0%），差异均有统计学意义（P<0.05）。 结论 结肠癌患者术前RDW水平与术后3年和5年存活率有一定相关性，可作为判断结肠癌术后存活率的一个重要指标。
Abstract:
Objective To explore the correlation between the red cell volume distribution width (RDW) and postoperative survival rate of patients with colon cancer. Methods Total 119 patients with colon cancer who received surgical treatment from Januray 2011 to December 2013 in The Affiliated Huaian No.1 People’s Hospital of Nanjing Medical University were enrolled. Total 119 cases with ulcerative colitis (UC) at the same period in our hospital were randomly selected as control group to compare the difference of RDW in peripheral blood. Meanwhile，colon cancer patients were divided into the higher RDW group and the lower RDW group by the mean of preoperative RDW. The correlation between RDW and postoperative survival was analyzed based on long-term follow-up of patients with colon cancer. Results RDW in patients with colon cancer was higher than that of UC patients[（43.48±4.06）% vs (41.76±4.93)%，P<0.01]. Preoperative RDW of colon cancer was positively correlated with CEA（r=0.748，P<0.01）. RDW was not associated with tumor location，tumor size，pathological grading，tumor infiltration depth，postoperative pathological lymph node involvement（P>0.05）,and the survival rate of one year after surgery（P>0.05）. However，the survival rate at three years and five years after surgery in preoperative higher RDW group was significantly lower than that of lower RDW group. The survival rate at three years after surgery of the two groups were 68.6% and 85.3%，and the survival rate at five years were 40.0% and 75.0%，respectively. The differences were statistically significant (P<0.05). Conclusion In higher preoperative RDW patients with colon cancer，the survival rate at three and five years after surgery was lower. Preoperative RDW may be a novel indicator for postoperative survival of colon cancer.
