摘要:
目的探讨透明帽辅助无痛肠镜下聚桂醇泡沫硬化剂对内痔治疗的疗效。方法前瞻性纳入50例于2021年1月至2021年12月就诊于东南大学附属中大医院江北院区消化科Ⅰ~Ⅲ度出血性内痔且符合纳入标准患者，并按照完全随机法分为泡沫硬化剂组及传统液体硬化剂组。泡沫硬化剂组采用透明帽辅助无痛肠镜下聚桂醇泡沫硬化注射疗法，液体硬化剂组按照常规硬化注射。比较2组注射点数、注射剂量、术中注射部位局部出血、术后并发症、治疗后短期有效率等指标。结果2组患者硬化注射点数、术后出血及肛门部烧灼感差异均无统计学意义（P=0.102、P=0.602、P=0.663）。2组患者治疗后短期有效率差异无统计学意义(P=0.814)；但泡沫硬化剂组的注射总剂量、术中局部出血及术后肛门部疼痛发生率均显著低于液体硬化剂组(P＜0.001、P=0.026、P=0.047）。结论无痛肠镜下聚桂醇泡沫硬化注射术在内痔治疗中注射剂量少，术中出血及术后疼痛发生率低，安全有效。
Abstract:
ObjectiveTo evaluate the treatment effect of internal hemorrhoids with lauromacrogol foam sclerotherapy assisted by cap-assisted painless colonoscopy.Methods50 patients who were confirmed with grade I-III bleeding internal hemorrhoids and met the inclusion criteria from January 2021 to December 2021 were enrolled.Patients were randomly divided into foam sclerotherapy experimental group and traditional liquid sclerotherapy control group.Both all received internal hemorrhoids injection under painless colonoscopy.The injection points, injection dose, intraoperative bleeding of injection site, postoperative complications, postoperative short-term effective rate of the two groups were compared.ResultsIt made no sense in comparison of sclerotic injection points, postoperative bleeding and anal burning between the two groups (P=0.102，P=0.602，P=0.663).There was no significant difference in shortdated treatment efficiency between the two groups(P=0.814).Compared with liquid sclerotherapy group, foam sclerotherapy group had lower injection total amount, lower incidence of intraoperative bleeding of injection site and postoperative anal pain (P<0.001, P=0.026, P=0.047).ConclusionLauromacrogol foam sclerotherapy under painless colonoscopy is safe and effective in the treatment of internal hemorrhoids with low injection dose, low rate of intraoperative bleeding and postoperative pain.
